We are limiting the footfall into the health centre in order to protect both our staff and patients. We are offering telephone triage daily and the doctor will determine when, how and where you will be treated. Please ring before 10.30am.

Anyone wishing to register with the practice must live within 7.5 miles of Portaferry Health Centre (as per google maps). Postal addresses of Portaferry, Ardkeen, Cloughey, Portavogie, Ballygalget & Ballycran.

If you are transferring within GP or NI, you will complete a HS200 form and if you are coming from outside the UK you will need to complete a HSCR1 form. You will also be asked to complete a patient lifestyle questionnaire. If possible, you should bring a copy of any repeat medication that you are currently prescribed.

Patients are registered with the practice and not an individual GP.

Please note you will be required to provide proof of address by way of a recent utility bill and one form of photographic ID such as a passport or driving licence.
